The purpose of this form is to advise the central administration (i.e. the Academic Affairs Office and the International Affairs and Public Relations Division) regarding an awarded Honor to an Institute's academic staff member.

You must supply the Academic Affairs Office with a copy of the original announcement regarding the honor. Please state herein if you will send a hard copy via internal mail to Aviva Ovadia in the Academic Affairs Office. Alternatively you may upload an electronic copy of the file:
